2. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) which can be downloaded at www.csc.gov.ph;
3. Work experience sheet (attachment to CS Form No. 212) which can be downloaded at www.csc.gov.ph;
4. Photocopy of Diploma/Transcript of Records.
5. Photocopy of PRC license/CSC eligibility
6. Certificate from previous employment (if applicable)
7. Certificate of training (if applicable)
